29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: On or about May 15, 2023, in Department #3 and Department #1, extension cords were being used that had damaged electrical insulation and were not removed from service exposing employees to electrical shock hazards in the following instances: a) In Department #3 Press Area, an orange 16 AWG extension cord damaged, it had numerous insulation cuts with exposed inner conductors used to supply power to the Central machinery 6" bench grinder machine with light 3450 RPMs, 120V, 2.1 A. b) In the Machine Shop Building, an orange 16 AWG extension cord used to supply power to phone charger had damaged insulation exposing inner conductors.
