29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about May 15, 2023, employees were exposed to electrical shock hazards due to electrical equipment not being used or installed in accordance with instructions included in listing or labeling in the following instances: a) In Department #3 Press Area, a lightweight brown 3-outlet extension cord connected to another 3-outlet white extension cord was daisy-chained and being used to supply power to a Microwave 1050W (8.75 A). b) In Department #7 Screw Machine Building, Relocatable Power Tab (RPT) permanently attached to wall supplying power to phone charger.
