29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Super Hog Pit- On or about September 21, 2023, employees performing maintenance tasks were exposed to amputation hazards from an unguarded woodchipper as a result of utilizing energy control procedures for the Super Hog that had not received a periodic inspection. b) Flitch Surfacer Area- On or about September 21, 2023, employees operating the Flitch Surfacer were exposed to amputation hazards from unguarded moving parts as a result of utilizing energy control procedures that had not received a periodic inspection. c) Line 4- On or about September 21, 2023, employees performing maintenance were exposed to amputation hazards from unguarded moving parts as a result of utilizing energy control procedures for Slicer #4 that had not received a periodic inspection.
