This violation was most recently observed, on or about February 8, 2022, at 20 Wilson Ave. Spring Valley, NY 10956: a-(West Side of Roof) Employee #2 was engaged in roofing activities on a 6 on 12 pitched roof, three story 9-unit residential building were exposed to fall hazards of approximately 30ft to the ground below as personal fall arrest system, guardrail system, or safety net were not being used.
