29 CFR 1910.178(a)(5): The user did not request, for trucks equipped with front-end attachments other than factory installed attachments, that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a. Site- The Hyster H90XLS forklift was operated with a 10 foot extendable boom attachment and did not have an identification plate attached showing the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. b. Site- The Hyster S55XM forklift was operated with a steel scale pan attachment and did not have an identification plate attached showing the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. c. Site- The Doosan GC32P-3 forklift was operated with a Darnell's Butt-Buster Manufacturing roll cutter attachment and did not have an identification plate attached showing the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. d. Site- The Clark CGP 25 forklift was operated with a Cascade clamp attachment and did not have an identification plate attached showing the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.
