29 CFR 1910.219(i)(2): Revolving surfaces of shaft coupling(s) were not covered by a safety sleeve: Floor III mixing area - On or about November 26, 2018, employees performing tasks such as but not limited to quality control, and loading, unloading and operating the mixing machine were exposed to caught-in and amputation hazards where the following rotating shaft couplings (with a 2-inch riveted collar, 1 3/4 inch diameter, 2 1/2 inch long) located 82-inches (6 feet-10 inches) above the floor were not guarded: a. serial number 05693 b. serial number 05692 Kumho Tire Georgia, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.219(i)(2), which was contained in OSHA inspection number 1196140, citation 1, item 10 issued June 6, 2017 and was affirmed as a final order on June 30, 2017, with respect to a workplace located at 3051 Kumho Parkway, Macon GA 31216.
