29 CFR 1904.29(b)(6): Employees names were not removed from the 300 Log when employee(s) experienced privacy concern cases: a) Conifer Care Communities dba Christopher House, as the exposing employer, did not remove employee names on the OSHA 300 Log for calendar year 2009: Entry #905 with an injury date of August 3, 2009, listed the employee name in Column (B) when it should have been recorded as a privacy case . On the employer's corrected form this is now indentified as Entry #904. Entry #907 with an injury date of November 8, 2009, listed the employee name in Column (B) when it should have been recorded as a privacy case . On the employer's corrected form this is now identified as Entry #905.
