29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. On or about October 17, 2018 and prior to, located on the elevated water tower storage tank system a powered single-point adjustable suspension scaffold was erected, moved, operated, maintained or inspected by employees who were not trained on the applicable standards or procedures to be followed to minimize scaffolding hazards associated with the work exposing employees to fall potentials in excess of 65 feet. V & T Painting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.454 (b), which was contained in OSHA Inspection Number 1004853, Citation Number 1, Item Number 13-b, Issued on 4/15/2015, with respect to a workplace located at 3599 Oxford Reily Rd. Oxford, OH 45056. This citation became final on 2/5/2016.
