29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: a) Starch Department- On June 6, 2018, the employer failed to ensure that the cord for the Doran 7000xl Scale was not connected or altered in such a way that preventing proper continuity. The plug for the scale was missing the grounding pin and the plugs hot and neutral pins were flipped which then reversed the polarity of the equipment. No abatement certification or documentation is required for this item.
