Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) 3000 Route 9W, Saugerties NY. - On or about January 21st, 2015 - Employees were exposed to fall hazards of approximately 21 feet in height while engaged in roofing work without the use of a guardrail system, safety net system, or personal fall arrest system. Paulo Miranda DBA Madeira Framing Co was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 945369, citation number 1, item number 4a and was affirmed as a final order on 02/10/2014, with respect to a workplace located at 126 High Barner Roadhighrose Ridge in Middletown NY.
