29 CFR 1910.146(e)(1):Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a) Boiler house, Woodchip/bark-chip storage area; On or about December 7, 2020 and at times prior to, the employer allowed employees to work in the confined spaces of the woodchip/bark-chip storage area without documenting the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit, exposing employees to caught-by (chain conveyor), entrapment, and engulfment hazards. Browder and Sons Veneer Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.146(e)(1), which was contained in OSHA inspection number 1506065 citation number 1 item number 1d and was affirmed as a final order on 4/21/2022.
