29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about August 12, 2021, and at times prior thereto, an employee was applying a lock and tag to equipment without being an authorized employee, exposing employees to hazards such as, but not limited to electric shock.
