29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about October 17, 2018 and prior to, located on the elevated water tower storage tank system a powered single-point adjustable suspension scaffold was utilized by employees who were not trained on the applicable standards or procedures to be followed to minimize scaffolding hazards exposing employees to fall potentials in excess of 65 feet. V & T Painting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.454 (a), which was contained in OSHA Inspection Number 1004853, Citation Number 1, Item Number 13-a, Issued on 4/15/2015, with respect to a workplace located at 3599 Oxford Reily Rd. Oxford, OH 45056. This citation became final on 2/5/2016. V & T Painting, LLC was also previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.454 (a), which was contained in OSHA Inspection Number 1007011, Citation Number 1, Item Number 10-a, Issued on 4/15/2015, with respect to a workplace Located at 1696 Millville Oxford Rd.
