On June 23, 2014, at a worksite near the Intersection of State Road 3003 and Berkhimer Lane, Claysburg, PA 16625, the employer did not furnish employment and a place of employment free from recognized hazards that are causing or likely to cause death or serious physical harm to employees, in that an employee operated a 2013 Polaris Ranger 800 6x6 utility terrain vehicle on a side-slope of approximately twenty-two (22) degrees without first inspecting the unit to ensure proper installation of cab nets.
