29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not marked in a manner that readily identified them as defective, tagged with "Do Not Use" or similar language, or were withdrawn from service until repaired. a) Jobsite: North side of structure - On or about 5/9/14, the twelve (12) foot portable aluminum ladder with a damaged rung, missing feet, and a split side rail was not marked or tagged as defective and withdrawn from service until repaired. Employees used the ladder to access the upper levels of the residential construction building. AMA construction was previously cited for a violation of this occupational safety and, which was contained in OSHA inspection number 973921, citation number 1, item number 3a, and was affirmed as a final order on 8/4/14, with respect to a workplace located at 46-54 Keegan Court, Evesham, NJ 08053
