29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about July 24, 2024, on the A-Line, employees were performing continuous vulcanizing, where the grounding prong was interrupting the equipment's path to ground in that: a) Two Profitter drum fans were missing ground pins. b) One Profitter drum fan was plugged into the outlet upside down.
