Text For Citation: 02 Item/Group: 005 Hazard:
29 CFR 1910.219(c)(4)(ii): The unused keyway(s) of projecting shaft end(s) were not filled up or covered: a) Meadow Gold Dairy, Gallon Case Machine: On or about June 14, 2022 and at times prior, employees were exposed to caught in hazards while working around an unprotected 1 inch diameter keyed shaft located on the Gallon Case machine in direct vicinity of the controls. The employer failed to ensure the keyed shaft presented a smooth edge. b) Meadow Gold Dairy, Palletizer: On or about June 14, 2022 and at times prior, employees were exposed to caught in hazards while working around the unprotected 2 1/2 inch diameter keyed shaft located on the palletizer. The DFA Dairy Brands Fluid, LLC (dba: Meadow Gold Dairy) was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.219(c)(4)(ii), which was contained in OSHA inspection number 1528324, citation number 1, item number 5/b and was affirmed as a final order on 10/18/21, with respect to a workplace located at 312 3rd Ave. South, Great Falls, MT 59405.
