29 CFR 1910.119(d)(3)(i)(B):The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process. a. PSI - On or about February 9, 2022, the employer failed to ensure that the P&IDs were accurate in that the AU 11 was removed from service out of second processing but still shown on P&ID, exposing employees to potential uncontrolled release of anhydrous ammonia. Failure to provide accurate P&IDs can lead to human error when operating or performing maintenance on ammonia refrigeration process equipment, exposing employees to fire, explosion, and toxic release hazards. b. PSI - On or about February 9, 2022, the employer failed to ensure that the P&IDs were accurate in that the P&ID for the receiver did not accurately reflect or document the presence of a pressure gauge to the pump out line in the engine room, exposing employees to potential uncontrolled release of anhydrous ammonia. Failure to provide accurate P&IDs can lead to human error when operating or performing maintenance on ammonia refrigeration process equipment, exposing employees to fire, explosion, and toxic release hazards. c. PSI - On or about February 9, 2022, the employer failed to ensure that the P&IDs were accurate in that the P&ID 9R16 did not accurately reflect operating conditions as it did not include valve HV17 on the roof, exposing employees to potential uncontrolled release of anhydrous ammonia. Failure to provide accurate P&IDs can lead to human error when operating or performing maintenance on ammonia refrigeration process equipment, exposing employees to fire, explosion, and toxic release hazards. d. PSI - On or about February 9, 2022, the employer failed to ensure that the P&IDs were accurate in that the P&ID did not include pump out valve into POV 22 on the roof ,exposing employees to potential uncontrolled release of anhydrous ammonia. Failure to provide accurate P&IDs can lead to human error when operating or performing maintenance on ammonia refrigeration process equipment, exposing employees to fire, explosion, and toxic release hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
