29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) Estate Custom, LLC located at or near 3451 Northup Ave, South Bloomfield, OH 43103: On and preceding December 14, 2021, the employer did not ensure that employees were protected from the hazard of cave-ins by an adequate protective system. An employee was performing water and storm plumbing installation in an excavation that was 18-20 feet deep working within a single 8' tall trench shield. The soil was fine sand in most spots and visibly coming loose throughout the inspection. The following condition exposed employees to struck-by/caught-in hazards. Estate Custom, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1471742 , citation number 1, item number 6 and was affirmed as a final order on June 22, 2020, with respect to a workplace located at Scioto Landing development, SOUTH BLOOMFIELD, OH 43103.
