29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a. Site-The Hyster H90XLS forklift was operated with a 10 foot extendable boom attachment that was not approved by the manufacturer. b. Site- The Hyster S55XM forklift was operated with a steel scale pan attachment that was not approved by the manufacturer. c. Site- The Doosan GC32P-3 forklift was operated with a Darnell's Butt-Buster Manufacturing roll cutter that was not approved by the manufacturer. d. Site- The Clark CGP 25 forklift was operated with a Cascade clamp attachment that was not approved by the manufacturer.
