29 CFR 1910.119(f)(1): The employer shall develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and shall address at least the following elements: 29 CFR 1910.119(f)(1)(iv): Safety systems and their functions: a. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 7.2: Safety Systems, the safety systems and their functions did not include the kettle temperature sensor and alarms, function/description of the alarm, and set point criteria for the temperature which initiated the alarm for PETN (pentaerythritol tetranitrate) and non-PETN mixtures. b. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 7.2: Safety Systems, the safety systems and their functions did not include the scrubber system sensor and alarm, function/description of the alarm, and set point criteria. c. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 7.2: Safety Systems, the safety systems and their functions did not include the steam pressure relief valve settings. d. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 7.2: Safety Systems, the safety systems and their functions did not include the agitator torque limiting clutch settings and its complete function/description. e. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 7.2: Safety Systems, the safety systems and their functions did not include the kettle shaft tach ring motion sensor and alarms, function/description of the system, and the set point criteria. f. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 7.2: Safety Systems, the safety systems and their functions did not include the E-stops and their functions/descriptions for each kettle. Austin Powder Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.119(f)(1)(iv), which was contained in OSHA inspection number 957166, citation number 1, item number 9b and was affirmed as a final order on August 21, 2014, with respect to a workplace located at 7LC10 Blandy Road, Camden, Arkansas 71701.
