29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: 101 North Main Street, Newark, Illinois, west building shop, SteelWeld 500 Ton mechanical press brake model no. M06-12 - on or about May 4, 2017, employees were exposed inadvertent contact with live electrical wiring at an electrical box that had a missing cover while picking up sharp, scrap metal pieces that were thrown on the floor in close proximity to exposed, energized wires. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Louis Dierzen dba Dierzen Sales, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 1125876, citation number 01, item number 005 and was affirmed as a final order on August 29, 2016, with respect to a workplace located at 101 N. Main St., Newark, IL 60541.
