29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Throughout the Plant - On or about 03/08/16 and at times prior thereto employees performed repairs, maintenance and re-tooling of equipment under lockout/tagout procedures which include (but are not limited to): Airlay #3 Bale Breakers, Resin Application Feed 1 and 2 (04/21/09). The employer did not conduct periodic and at least annual inspections of the written lockout/tagout procedures. Workers were exposed to electrical energy, steam, compressed air, hydraulic energy, hot surfaces and fluids, potential (gravitational) energy, crushes and amputations. HP Pelzer Automotive Systems, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 924666, citation number 1, item number 7, issued on 01/28/2014 and affirmed as a final order on 02/20/2014, with respect to a workplace located at 1884 Warrenton Hwy, Thomson, GA 30824. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
