29 CFR 1904.29(b)(3): Each Recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA 301 form or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness occurred: a) On or about September 13, 2011, Conifer Care Communities dba Christopher House, as the exposing employer, did not timely record the following injuries on the OSHA 300 Form: An injury occurring on January 7, 2011 was not recorded on the OSHA 300 for the Year 2011 within 7 calendar days of receiving information that a recordable injury had occurred. Entry #910 resulted in days of restricted activity. Column L (number of days of restricted activity) was blank. Column L should have been recorded as 59 days. An injury occurring on January 21, 2011 was not recorded on the OSHA 300 for the Year 2011 within 7 calendar days of receiving information that a recordable injury had occurred. Entry #912 resulted in both days away from work and days of restricted activity. Both Column K (number of days away from work) and Column L (number of days of restricted activity) were blank. Column K should have been recorded as 8 days and Column L should have been recorded as 180 days. Definitions: Column (H) is to record Days Away From Work. Column (I) is to record Days of Job Transfer or Restriction. Column (J) is to record Other Recordable Cases. Column (K) is to record the number of days away from work. Column (L) is to record the number of days with restricted activity or job transfer. Column (M)(1) through (M)(6) indicates if entry is an injury or one of 5 types of illness.
