29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) ADA Dash Main Line A900; On or about June 18, 2016 and at times prior thereto, Employee #15 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace welding tips on robots and allowed to clear sensor faults without locking out the electrical, hydraulic and pneumatic energy sources.
