29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry: a)240 North Reach Rd. Williamsport, PA 17701- On or about September 21, 2023, employees performing maintenance tasks were exposed to confined space hazards such as energized equipment, engulfment, oxygen-deficient atmospheres, and electricity inside of permit-required confined spaces without the employer implementing procedures for rescuing entrants from permit spaces.
