29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): a. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Perkins Junior E) which did not have its point of operation properly guarded. b. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Enco W) which did not have its point of operation properly guarded. c. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Rouselle Model OF, No. 184) which did not have its point of operation properly guarded. d. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Rouselle, Model OF, Serial Number 26420) which did not have its point of operation properly guarded. e. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Rouselle, Model OF, Serial Number 27383) which did not have its point of operation properly guarded. f. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Alva Fallen, Model BT 12-10) which did not have its point of operation properly guarded. g. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Rouselle #2, Serial No. 20721) which did not have its point of operation properly guarded. h. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a power press (i.e. Press P4, PPF-18) which did not have its point of operation properly guarded. i. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. 50 tons, no other info available) which did not have its point of operation properly guarded. j. On or about October 20, 2016, at the forming department of the above addressed jobsite, employees were exposed to a caught by hazard while working with a mechanical power press (i.e. Rouselle OF) which did not have its point of operation properly guarded. Pavilion Furniture Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 1910.217(c)(1), which was contained in OSHA inspection number 513638, citation number 1, item number 6 and was affirmed as a final order on October 3, 2012, with respect to a workplace located at 16200 NW 49th Avenue, Hialeah, FL 33014.
