29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: A) Centrex Plastics, LLC, doing business as American Plastics, located at 814 W Lima St, Findlay, OH 45840: On or about May 17, 2020, the employer did not ensure that employees used safe electrical work practices in order to be protected from electrical hazards such as, but not limited to, electric shock and arc flash. Employees were exposed to 480 volts electrical hazards while conducting activities such as troubleshooting, cleaning, repairing, and replacing electrical components such as, but not limited to, wires for the heaters of the Toshiba Plastic Injection Molding Press # 51 (Model: ISF720SI-59A; Serial # 415705).
