29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals: a. On or about October 20, 2016, at the forming department of the above addressed jobsite, a mechanical power press (i.e. Alva Fallen, Model BT 12-10) had its pedal unprotected, exposing employees to a caught by hazard. Pavilion Furniture Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 513638, citation number 1, item number 5 and was affirmed as a final order on October 3, 2012, with respect to a workplace located at 16200 NW 49th Avenue, Hialeah, FL 33014.
