29 CFR 1910.219(c)(3): Vertical or inclined shafting seven -7 feet or less from floor or working platform was not enclosed with stationary casing(s) (a) On or about September 16, 2013 the inspection revealed that employees were exposed to the vertical rotating shaft on the left side of the vertical lathe located in the east bay of the John Day dam. (b) On or about September 11, 2013 the inspection revealed that the employer did not provide adequate warning signs and/or barriers to keep unqualified personnel away from the rotating vertical shafts of the power generation turbines. Located at the machine shop floor level in the John Day dam. (c) On or about September 11, 2013 the inspection revealed that employees were exposed to serious injury from the vertical rotating shafts on the turbine governors, located at level 3 of the John Day Dam. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 741082, notice number 1, item number 2 and was affirmed as a final order on April 24, 2013, with respect to a workplace located at The Dalles Dam in The Dalles, Oregon. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 96892, notice number 1, item number 5 and was affirmed as a final order on September 15, 2011, with respect to a workplace located at McNary Dam in Umatilla, Oregon.
