29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose: a) for the establishment located at 2401 Airport Road Plant City (FL), where employees were using an altered electrical cord that was coming directly from an electrical panel to a Bridgeport Milling Machine for supplying power by splicing the power cord of the milling machine and taping the two connections together, on or about July 19th, 2013.
