The employer does not establish and implement written procedures to maintain the on-going mechanical integrity of process equipment: 29 CFR 1910.119(j)(2): The employer did not establish and implement written procedures to maintain the on-going mechanical integrity of process equipment: UPSTATE NIAGARA COOPERATIVE, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312535800, CITATION NO.1, ITEM NO.7a, AND WAS AFFIRMED AS A FINAL ORDER ON 02/13/09, WITH RESPECT TO A WORKPLACE LOCATED AT 1730 DALE ROAD CHEEKTOWAGA, NY, 14225. A recent example of this occurred on 10/25/11, when: a) In the establishment, Employer did not establish and implement a written procedure for installing and changing flange valves such as, but not limited to: Parker Inlet Pressure Regulator Type A4A that included torque specifications for the bolts. b) In the establishment, Employer did not establish and implement a written procedure for what steps to follow once a Pressure Relief Valve (PRV) is lifted during service. c) In the establishment, Employer did not establish a written procedure for periodic inspections and testing of high level alarms and high level emergency shutdown systems on process equipment such as, but not limited to: LP Accumulator, Blow Mold Chiller, Control Pressure Receiver, Ice Builder #1, Process Chiller and Suction Accumulator Cooler #3. To abate this violation, the employer shall establish and implement written procedures for installation, replacement and maintenance of flange valves and Pressure Relief Valves. Also, the employer shall establish and implement written procedures for periodic inspections and testing of refrigerating systems following IIAR Bulletin No. 110 and Manufacturers' Instructions. Pursuant to 29 CFR 1903.19(d), the employer must submit documents describing the steps it is taking to ensure compliance, within 30 days of the date it receives this citation. ABATEMENT DOCUMENTATION REQUIRED
