Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which are free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the foot activation pedal was not guarded against unintended operation from falling or moving objects or by accidentally stepping onto the pedal thereby exposing employees to crushing and amputation hazards on the following machines: a) ME 8053, Verson press brake b) ME 8011. Verson press brake c) ME 7183, riveter d) ME 8050, Chicago press brake Among other methods, one feasible and acceptable abatement method to correct this hazard is to guard the foot pedals. MESTEK INC. WAS PREVIOUSLY CITED FOR AN EQUIVALENT VIOLATION, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 102794021, CITATION NUMBER 1, ITEM NUMBER 3C, AND CITATION TWO, ITEM 5, ISSUED ON 08/31/98, WITH RESPECT TO A WORKPLACE LOCATED AT S. WINDSOR CT. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIREDATION
