29 CFR 1910.119(m)(6):The report was not reviewed with all affected personnel whose job tasks are relevant to the incident findings including contract employees where applicable: (a) Cargill, Inc. DBA Cargill Meat Solutions Corporation, 1505 East Burlington Avenue Fort Morgan, CO 80701: On or about February 6, 2025, the employer failed to review the incident findings with affected personnel following an ammonia release. Despite the discovery of active ammonia leakage due to pipe corrosion, no documented review of the incident findings was conducted with maintenance staff, refrigeration operators, or contract employees involved in system repair and inspection. (b) Cargill, Inc. DBA Cargill Meat Solutions Corporation, 1505 East Burlington Avenue Fort Morgan, CO 80701: On or about February 10, 2025, the employer failed to review incident findings with affected personnel following a significant ammonia release from a pressure relief valve (PRV) on the sub-roof liquid header. The PRV failed to reseat, resulting in a discharge of ammonia and necessitating hazmat PPE, evacuation procedures, and emergency system shutdown. No documented review of this incident was completed with refrigeration technicians, maintenance workers, or contractors responsible for PRV servicing and startup procedures.
