29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from the recognized hazard of damaged insulation. On or about October 16, 2024, in the transformer area, employees utilized a high voltage cable coming out of the transformer and leading into high voltage circuit box which had outer insulation damage while the cable was being used to supply power to equipment on location.
