29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to items (d)(3)(i) through (d)(3)(vi) of this paragraph. a) Boiler house, Woodchip/bark-chip storage area; On or about December 7, 2020 and at times prior to, the employer allowed employees to work in the confined spaces of the woodchip/bark-chip storage area without developing and implementing the means, procedures, and practices necessary for safe permit space entry operations, exposing employees to caught-by (chain conveyor), entrapment, and engulfment hazards. Browder and Sons Veneer Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.146(d)(3), which was contained in OSHA inspection number 1506065 citation number 1 item number 2 and was affirmed as a final order on 4/21/2022.
