29 CFR 1915.89(b): The employer did not establish and implement a written program and procedures for lockout and tags-plus systems to control hazardous energy during the servicing of any machinery, equipment, or system in shipyard employment to include requirements of items 1 through 6 of this paragraph: a) Quality Coatings of Virginia: On or about 03/22/2022, the employer failed to establish and implement a written lockout and tags-plus system to control hazardous energy when working in tanks and other spaces where piping systems are used to release substance into the area. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
