29 CFR 1910.147(c)(5)(ii)(D):Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): a. On the roof - On or about February 9, 2022, their was a lockout device on FUT LV Valve but no tag or other means of identifying the individual that locked out the valve was present. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
