29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: (a) Worksite: On or about 9/26/2016, employees were exposed to electrical hazards from using damaged extension cords to power machinery throughout the facility. West Warwick Welding, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.334(a)(2)(ii), which was contained in OSHA inspection number 1187975, citation number 1, item number 4-f, and was affirmed as a final order on 12-22-2016, with respect to a workplace located at 970 Main Street West Warwick, RI 02893.
