29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). Location: Worksite Employees are being exposed to struck-by, caught between, and other hazards associated with excavations while working in foundation excavation with straight cut earthen walls, by not providing adequate protective systems designed in accordance with 29 CFR 1926.652 (b) or (c). The employer routinely fails to provide protection for employees engaged in excavation work as demonstrated on August 13, 2021, at the 18 Crestway Road worksite where employees were exposed to excavation hazard injuries
