29 CFR 1910.178(l)(1)(i): The employer had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (l): a. At this facility located at 1125 Cranleigh Ave, Deland, Florida; On or about April 14, 2021, the employer exposed employees to struck-by and crushing hazards, when employees were allowed to operate powered industrial forklifts without first completing the training required by this standard.
