29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a. On or about 12/12/2019, at the fabrication area loading door platform, employees were exposed to a tripping hazard, in that, a floor hole at the west side of the platform measured to be 8 inches by 6 inches located in front of the stairway was not covered.
