29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: a) On or about July 26, 2018, employees engaged in masonry work were not protected from the hazards of falling approximately 30 feet to the ground while working from a tubular welded frame scaffold platform. Polo Masonry Builders, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1001826, citation number 2, item number 2 and was affirmed as a final order on January 29, 2015, with respect to a workplace located at 2829 N Leavitt Ave., Chicago, Illinois 60647. Polo Masonry Builders, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 108513, citation number 1, item number 2 and was affirmed as a final order on December 13, 2011, with respect to a workplace located at 3634 W. Montrose Ave., Chicago, Illinois 60618. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
