29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a. On or about 1/16/2018 at the establishment located on 5911 E. Dr. Martin Luther King Junior Blvd., Tampa, FL, employees conducted a permit-required confined space entry into a chemical transporting tank containing a hazardous atmosphere created by carbon monoxide, and the employer had not prepared an entry permit. The Tampa Bay Truck Wash, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1021462, citation number 1, item number 8b and was affirmed as a final order on 7/21/2015, with respect to a workplace located at 5911 E. Dr. Martin Luther King Junior Blvd., Tampa, FL.
