29 CFR 1910.1030(f)(2)(iv): The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A: (a) Funeral Home - A review of the employer's written Exposure Control Plan, on or about December 17, 2017 indicated that the plan, which employees having occupational exposure were covered by, listed November 16, 2010 as its last review date. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
