29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Throughout facility: The employer did not ensure that employees operating a powered industrial truck in the facility, including, but not limited to, a Harrington Manufacturing Company lift, model 8F8000T serial number 3111, were competent to operate the lift as demonstrated by an evaluation of the employee's operational capabilities, on or about 8/11/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
