29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) Main Fan Room: On or about July 13, 2017 and at times prior to, the employer exposed employees to electrical burns, shock and fire hazards from a 4 to 20 milliamp electrical box housing signal feedback wiring that was not effectively closed. b) Main Fan Room: On or about July 13, 2017 and at times prior to, the employer exposed employees to electrical burns, shock and fire hazards from a 4 to 20 milliamp electrical box housing signal fan bearing temperature relay wiring that was not effectively closed. c) Bag House Area: On or about July 13, 2017 and at times prior to, the employer exposed employees to electrical burns, shock and fire hazards from a 24 volt electrical box housing control wiring to rotary valve that was not effectively closed.
