29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about April 14, 2017, and times prior thereto at Plant #3; specific procedures were not utilized for the control of hazardous energy for the employer's machine and equipment at: (a) Tunnel Kiln Entry- HECP and SOP 0065; (b) Tunnel Kiln Exit- HECP and SOP 0035; (c) Clay Prep and Clay Sheds, SOP and HECP 0025; (d) Green Unloading Zone, SOP and HECP 0045; (e) Roller Green "Symbol" System SOP and HECP 0060; (f) Primary Clay Prep and HECP 0075; (g) Roller Kiln Dryer Exit and Kiln Entry SOP 0020 and HECP 0015; (h) Roller Kiln Exit and Symbol Red SOP and HECP 0015; (i) Redside Unloading Zone SOP 0070 and HECP 0070; (j) Roller Presses RP-1 to RP-5 SOP and HECP 0010; (k) Roller Packaging 1-5 SOP and HECP 0055; (l) Tunnel Dryer Exit SOP and HECP 0040; (m) Shrink Wrap Machine SOP and HECP 0030; (n) Tunnel Green Symbol System SOP and HECP 0050; (o) Tunnel Presses TP-1 to TP-4.
