29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: The employer does not ensure fall protection anchor points are installed to support at least 5,000 pounds per employee. The lack of fall protection anchors was most recently observed on 8/4/2016 at the jobsite of a new construction project located at about 2150 Tuscany Ridge Ct, Maryville, IL 62062. The employees conducting residential roofing operations are exposed to the hazard of falling approximately thirteen (13) feet.
