Text For Citation: 02 Item/Group: 004 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Plant, DS Area - On or about 03/08/16 and at times prior thereto employees performed maintenance/repairs on the robotic water jet cutter. The employer did not develop, implement, and utilize a written lockout/tagout procedure to protect workers from moving parts, 480 V electric power, compressed air, and high pressure water jets. b) Plant, BMW Foam Clamp Area - On or about 03/08/16 and at times prior thereto employees performed troubleshooting of foam clamps #1, #2, #3 and the surrounding equipment, and the employer did not develop, implement, and utilize written lockout/tagout procedures to protect workers from moving parts, 480 V electrical power, hydraulic power, and compressed air. c) Throughout the Plant - Employees conducted repairs/maintenance of equipment including (but not limited to) the Laroche Tearing Machine, the Baler, Presses in the hot Mold Area, Oven Presses in the Light Weight Foam Molding, Presses in the Light Weight Foam Molding, and additional equipment subjected to lockout/tagout as described in the company inventory of date 04/13/16. The employer did not utilize written lockout/tagout procedures, exposing employees to electrical energy, steam, compressed air, hydraulic energy, hot surfaces and fluids, potential (gravitational) energy, crushes and amputations. HP Pelzer Automotive Systems, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 924666, citation number 1, item number 5, issued on 01/28/2014 and affirmed as a final order on 02/20/2014, with respect to a workplace located at 1884 Warrenton Hwy, Thomson, GA 30824. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
